What the form records, and why each field is there

AS/NZS 3760:2022 changed what a good record looks like. The 2022 edition asks service providers to record the actual measurement for each test (earth continuity resistance, insulation resistance, leakage current) rather than a bare pass or fail, so the form below has a number field for every reading. In Australia keeping records is recommended rather than mandatory under the standard, but a register with readings is what an auditor, insurer or principal contractor will ask you for.

In Queensland the tag itself also has rules. For service, office, manufacturing, amusement and rural work, the Electrical Safety Regulation 2013 requires the tag to show the prescribed details: the test date or the retest date, plus the tester's name or a licence number. The form captures both so the tag and the register always agree.

How it fits ServiceM8

A plain form works once per item: the tester fills it in again for each appliance. The better setup is ServiceM8's Asset Management add-on. Pick the interval before you write the due date

The due date depends on the environment, not the appliance. AS/NZS 3760:2022 Table 2.4 runs from 3 months on construction sites to 5 years for equipment that is not flexed or open to abuse, and Queensland adds its own minimums for some work types. The form asks for the environment first and then the interval, so the tester has to make that call on site. Readings and limits to keep in view

Common pass limits cited from the standard are an earth continuity resistance not above 1 ohm and an insulation resistance of at least 1 megohm. For RCDs the standard sets a maximum trip time by type, and the form records the measured trip time in milliseconds so the result can be checked later. Put these limits in each question's note so a new tester sees them while testing, and check them against your own copy of the standard, because Standards Australia sells it and summaries can lag.
